Questionnaire on sexuality and
smoking
Please answer even if you have never
smoked
Read
this first:

The aim of this study is to better understand the
links between smoking and sexual function, a topic that is not well documented.
In particular, we are interested in studying the effects of smoking cessation on sexual function. By participating in this survey, you contribute to enhancing
scientific knowledge on this topic.

To participate, you must be at least 18 years
old.

You will be asked detailed questions about your
sexuality. If you feel uncomfortable about answering these questions, you may choose
not to participate.

We will cover and compare current and former as well as people who have never smoked, so please answer even if you are a non-smoker. You will need 10-15
minutes to complete this questionnaire.

If you wish to, you will be invited after one week, 2 and 12
months to answer follow-up surveys (total 4 surveys
including this one), participation is voluntary and you can stop participating
at any time without having to give any explanation.

Your responses will be stored in a computer file
for statistical analysis, and will not be transmitted to any third party.
However, the people in charge of the ethics committee and the medical research
authorities in Switzerland will have the right to access the data, subject
to confidentiality.

We will ensure a high security level for recorded
data, particularly to minimize the risk of access by hackers.

If you do not want your answers to be kept on file,
please do not respond.

This study is independent of any industry
(pharmaceutical, medical devices, tobacco, etc.), and was approved by the
ethics committee of the Geneva University Hospitals.

Results of this study will be published in a scientific
journal.

The study begins in May 2011 and ends in December
2013, we expect 2000 participants.
